As part of our growth, Heritage Home – our Skilled Nursing facility – is expanding to its new home with a 40-bed Skilled Nursing Unit and a brand-new 10-bed Memory Care Unit, designed to provide compassionate, resident-centered care.

Summary Description:
Assists the Activities Manager with planning and conducting small and large group activities for resident population. Arrange volunteer chaperones and participates with resident population.

Supervision Received: Reports to the Activities Manager

Responsibilities:
1. Reports to the Activities Director. Demonstrates ability to lead group or individual activities.

2. Assists activity leader(s) when there is a large group setting.

3. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

4. Documents attendance and room program activities.

5. Remains with residents in activity setting until relieved by another appropriate person.

6. Demonstrates ability to control or diffuse disruptive or abusive behavior when demonstrated by a resident or other in a group or individual setting.

7. Implements scheduled activities in the appropriate place and time.

8. Completes all 1 on 1 visits each shift and records activity participation for the 1 on 1 daily visits.

9. Assist residents with shopping needs and provides transportation for those capable of shopping for themselves.

10. Reports any changes in residents' condition to a nurse immediately.

11. Assist bed-bound residents with reading and writing needs, errands, etc.

12. Is available to aid in evening and weekend activities.

13. Communicates the mission, ethics, and goals of the hospital.

14. Complies with all organizational policies regarding ethical business practices. Represents the organization in a positive and professional manner.

15. Demonstrates responsibility for knowing and adhering to all District and departmental policies and procedures.

16. Understands the importance of patient confidentiality and HIPAA rules and regulations.

17. Other Duties as assigned.

Qualifications:
1. High school graduate or GED.

2. CNA preferred.

3. Valid DL with a clean driving record.

4. Possesses current BLS certification.

Knowledge, Skills, and Abilities:
1. Demonstrates ability and willingness to work with older adults with different physical, mental, emotional, and social handicaps.

2. Good written and verbal communication skills,

3. Demonstrates evidence of effective problem resolution skills and the ability to maintain composure in stressful situations.

4. Ability to coordinate and conduct meetings, set and achieve goals.

5. Demonstrates the ability to be organized and efficient.

Working Conditions:
1. Subject to varying situations including ability to understand and respond appropriately, effectively, and sensitively to special population groups, including those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, etc.

2. Frequent contact with a variety of people including prisoners and people with mental, emotional, and physical disabilities.

3. Medical office, exam room, laboratory, radiology, emergency room and professional practice settings. Frequent exposure to communicable diseases and other conditions common to rural clinic environments.

4. The work environment involves exposure to unusual elements, potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.

5. May require travel between District sites.

Physical Demands:

Varied activities include standing, walking, reaching, bending, and lifting. Requires full range of body motion including handling and lifting patients. Also requires manual and finger dexterity, and eye-hand coordination. It requires standing and walking for extensive periods of time. Occasionally lift and carry items weighing up to fifty pounds. Requires corrected vision and hearing to normal range. This position frequently requires working under stressful conditions and irregular hours. Year-round highway travel between clinics and other facilities throughout the district is required.

Pay: Dependent on Experience.

Nature lovers will appreciate the nearby alpine lakes, perfect for fishing, water sports, and relaxation. Pinedale is also on the Path of the Pronghorn, where you can witness one of the last great animal migrations in the Western Hemisphere.
