Mission Coordinated Care of Sublette County (CCCS) will work to ensure every County resident or worker will receive the Human Services he or she needs in an atmosphere of mental, physical, spiritual and legal safety for all concerned; individuals, families, and service providers.
Purpose The Coordinated Care of Sublette County (CCCS) includes any of the community’s Human Service providers with an interest in optimizing Human Service delivery through enhanced inter-organization coordination. The purpose of CCCS is to ensure resources are used efficiently and that services are client driven without barriers or stigmas. Through greater mutual understanding and programming coordination, the current available resources including, but not limited to information, professional capabilities and funding, can be fine-tuned to meet the immediate and future needs of the community. 3.
