The Sublette Center is the only community-based, non-profit skilled nursing facility in Wyoming with a CMS (Centers for Medicare and Medicaid Services) 5 STAR rating.

The Sublette Center in Pinedale hosted the June Sublette County Chamber of Commerce Mixer on Thursday, June 17th. The Center offered a chance to socialize, meet the staff, and take a tour of the facility.

The Sublette Center is located at 333 Bridger Avenue in Pinedale. The Center offers independent living retirement apartments where residents can enjoy independent private apartment living without the worries of maintaining a home.

Room options are a studio apartment, 1 bedroom or 2-bedroom apartments. Each apartment is unfurnished and has wall-to-wall carpeting, draperies, large closets, soundproof walls, fireproof walls and individually controlled heating. TV and internet options are available. Apartment bathrooms have safety handrails in tub and shower, large wall mirrors, emergency call lights, vanity with sink, and quality fixtures. Most one bedrooms have the option of updating to a walk-in shower.

The Sublette Center offers many organized daily social and community activities in a clean, friendly, and caring environment.

Each month they have a building-wide birthday party for all the residents who have birthdays that month.

The Medical Wing is for patients with more intensive care needs. Licensed professional staff provide daily 24-hour nursing care, administration of medications, and assistance in daily living tailored to individual resident's needs for both short and long-term care. All facilities are on one floor and are designed for handicap accessibility.

The Sublette Center is extremely proud of the fact that they are the only community-based, non-profit skilled nursing facility in Wyoming with a CMS (Centers for Medicare and Medicaid Services) 5 Star rating.

The Center works in partnership with Sublette BOCES adult learning classes to offer practical experience residency for students who are working to obtain their CNA (Certified Nursing Assistant) qualification. After classroom work, students can work at the Sublette Center to get their required hands-on training and experience in a accredited facility, which is necessary to become CNA certified.

Three rooms we weren't able to take pictures of were the Medical Wing dining room, which was in the process of being cleaned up, the TV room in the Medical Wing, and the Heritage meeting room on the apartment side of the building.

Nice Landscaping	Activity Room This activity room has a table for card games, an exercise bike, books and magazines to read, and a pool table.	Servery The buffet line has a choice of menu offerings.

Walk-in Freezer & Refrigerator	Hallway Artwork Artwork in the building was done either by residents or people associated with the facility. Many are original paintings and drawings.	Laundry Residents can do their own laundry, or opt to have laundry done for them as an extra service.
Main Lobby	Sitting Nook One of several cozy sitting nooks located near inviting windows. There are magazines to read on the coffee table.	Security For security, outside doors are locked between 8:00 PM and 5:00 AM.

Bathroom Studio apartment bathroom.	Nurses pull cords Call cords are located in the apartments and bathrooms so nurses can be summoned, if needed.	Bird Cages There are several bird cages in the lobby with colorful and chatty birds.
Bird	Mailboxes Apartment residents can have their own mail boxes. Mail is delivered to boxes Monday through Saturday.	Medical Wing Room Rooms in the medical wing can be private or semi-private room which is shared with someone else.
Bathroom Bathrooms have handrails and plenty of room to move around.	Bird Feeder All of the outside windows have something to look at, such as a bird feeder.	Ice Machine The handy ice machine means they can always provide cold water for drinks.
Hair Salon There is a hair salon in the medical wing to take care of patient's hair styling needs.	Scales There are scales to monitor weight measurements.	Medical Wing Bathing
Whirlpool Tub This whirlpool tub is a favorite of the residents. Very relaxing. The special lift chair makes it easy and comfortable for patients to get in and out for bathing.	Cameras Cameras are located throughout for building security. Some special care patients in the medical wing wear electronic bands that will trigger an alarm if they go outside or through a door they aren't supposed to.	Sun Room The Sun Room in the Medical Wing has tables for activities, classes, entertainment and more.
Piano & Puzzles This activity room has a piano and puzzles, among many other things to do. Someone even comes in and gives piano lessons. There are 3 pianos in the building.	Chapel-Meeting Room This room serves as a meeting room and a small chapel, open 24-hours.	Outside Dog Kennels The outside kennels are a place to put the resident dogs and guest dogs when they need to be put out of the way. Resident dogs are allowed to roam through the public areas of the facility, and residents adore them.
Raised Gardens There are several raised garden beds on one of the outside patios. One year residents even successfully grew corn in them!	BBQ Patio The Center holds BBQs and outside functions out on this patio.	Outside Patio One of several outside patios. Residents can sit here and socialize with friends, enjoy the sunshine, have a cigarette (no smoking inside the building), and relax.
Outside Patio Another nice, quiet outside patio.	Front Parking Apartment residents have parking close to their building.	Rendezvous Pointe Rendezvous Pointe Senior Center is conveniently located right aross the street from the Sublette Center.
Proactive Rehabilitation Proactive Rehabilitation is located right across the street for physical therapy.	2 Busses The Center has a car and two busses to use to take residents on outings. Once a month they go to Rock Springs or Jackson for shopping. Once a week they go to Ridleys (half a block away) for shopping. All summer long they take Sunday drives to scenic locations around the valley.
