An independent consultant contracted by the Sublette Board of County Commissioners has mailed a questionnaire to 400 randomly selected Sublette County residents regarding a proposal to expand the Pinedale Clinic in order to obtain a Critical Access Hospital designation. If you have received this questionnaire in the mail, please fill it out and return it in the postage paid envelope provided.

The questionnaires are numbered to ensure that they have not been sent out in duplicate nor copied and submitted as reproductions. The numbers also facilitate analysis once the completed questionnaires are returned. To maintain the anonymity of each questionnaire response, our consultants did not link the questionnaire number with personal identifying information.

In addition to the 400 randomly selected questionnaires, the same questionnaire is available on the Sublette County website so that all interested residents can participate (www.sublettewyo.com).

The information gathered through this questionnaire will help inform the Commissioners on the opinions of Sublette County residents regarding important health care decisions facing Sublette County.
