Public Health Nursing is sponsoring the 2017 Pinedale Health Fair on Friday, September 22nd from 4:00 – 7:00PM at Rendezvous Pointe.
Blood Screenings available are:
Health Screen/TSH/Direct LDL/Ferritin $35.00
CBC Complete Blood Count (Blood Health/Immunity) $20.00
PSA (Prostate Specific Antigen) $30.00
Hemoglobin A1c (Diabetes Screening) $30.00
Vitamin D Screening (D2, D3, and Total Amt) $40.00
Vitamin B12 and Folate $40.00
Gluten Intolerance/Celiac Screening $45.00
Male Testosterone $45.00
Female Testosterone $45.00
InSure FIT Colon Screening Kit $25.00
Remember to:
- Fast 12 hours (Diabetics don’t Fast)
- Drink Water
- Take Prescribed Medications
- Wear loose sleeved clothing
Walk-Ins welcome after 8AM.
Blood Draws will be held before the Health Fair at these locations and times:
Monday, August 28th: Bondurant Fire Hall 7:00AM – 9:00AM
Tuesday, August 29 – Thursday, August 31, Rendezvous Pointe 6:00AM - 9:30AM
Please register for your blood draw online between July 31st and August 25th by clicking on this link or calling Sublette County Public Health at 307-367-2157 or 307-276-3575. This number may also be called if you need to schedule an appointment after August 25th.
Postcards have been mailed out to past blood screening participants. Bring the postcard or your blood draw confirmation with you to the Blood Draw for easy registration. You will receive the My Guide to Health Booklet from Quest Diagnostics (mailed in 14 business days after postcards sent out). Please open this mailing, it is not junk mail. These are your personalized results with educational information and tips regarding improving and maintaining your wellness. Additionally, you may set up an account and receive your Health Screening results through the MyQuest Patient Portal in 5-7 business days.
