Nominate your Nurse

Sublette County Health Foundation encourages people to share their stories to honor local nurses. The DAISY Foundation DAISY Award is given each year in recognition of those providing exceptional nursing care.

"We are proud to partner with the DAISY Foundation to honor the compassionate care of our extraordinary nurses throughout the year," said the Sublette County Health Foundation.

2025 DAISY Award nomination forms are available at either Library, either Clinic, or at the Sublette Center in Pinedale. Forms can be returned to either clinic or the Sublette Center. Forms must be returned by April 30th.
